Revoke address consent: Health Card

Withdraw your address consent for your spouse or common-law partner to update your address.

Use the Revoke Address Consent Form to withdraw your consent to authorize your spouse or common-law partner to update your address for your Health Card information.

Once the address consent withdrawal is registered:

  • any address change made by your spouse or common-law partner will not apply to you and your registered address
  • you can only enquire and update information related to your dependants if you’re their dependant primary contact

Immediate consent

If you need your address consent withdrawal to take effect immediately, contact MSI Resident Services to register your decision.

Changing your consent decision

It’s your choice. You can change your consent decision at any time.

Who can use this form

You can use this form to withdraw your address consent authorization if:

  • you’re a Canadian citizen or permanent resident and need to change your consent decision
  • you’re not the spouse or common-law partner of a study or work permit holder

What you need to do

  1. Complete the Revoke Address Consent Form (PDF).
  2. Submit your completed form.

If you need your consent decision to take effect immediately, contact MSI Resident Services to register your decision.

How long it takes

Your Health Card information is updated right away if you contact MSI Resident Services to revoke your address consent authorization. It should take 4 weeks to update your Health Card information if you submit your address consent withdrawal online or by mail, fax or secure drop box. It can take longer if more information is needed or if your form hasn’t been filled in correctly.

Cost

There is no cost to revoke address consent authorization.
