Dependant primary contact change: Health Card

Change the primary contact for your dependants who are 18 or younger. Dependants who are 18 or younger need to have 1 adult in their family registered as the dependant primary contact for their Health Card.

Dependants who are 18 or younger need to have 1 adult in their family registered as the dependant primary contact for their Health Card.

Persons 16 to 18 who apply for a Health Card as an individual or are already registered as an individual in the Health Card Registry don’t need a dependant primary contact.

A dependant can only have 1 dependant primary contact (the same contact is registered for all dependants in the family). You can change the primary contact at any time.

When both spouses or common-law partners provide address consent authorization, either person can update their dependant’s Health Card information including the address.

Immediate contact change

If you need the dependant primary contact change to take effect immediately, contact MSI Resident Services to register your change.

Who can use this form

You can use this form to change the primary contact for your dependants if you’re the registered dependant primary contact in the Health Card Registry for the dependants.

If 2 persons share the responsibility for a dependant and have not provided address consent authorization or are not registered together as a family in the Health Card Registry, contact MSI Resident Services to confirm the dependant primary contact.

What you need to do

  1. Complete the Dependant Primary Contact Change Form (PDF).
  2. Submit your completed form.

If you need the contact change to take effect immediately, contact MSI Resident Services to register your change.

How long it takes

Your Health Card information is updated right away if you contact MSI Resident Services to change the contact. It should take 2 weeks to update your Health Card information if you submit your change online or by mail, fax or secure drop box. It can take longer if more information is needed or if your form hasn’t been filled in correctly.

Cost

There is no cost to update the dependant primary contact.
