Address consent: Health Card
Authorize your spouse or common-law partner to update your address.
Use the Address Consent Form to authorize your spouse or common-law partner to update your address for your Health Card information.
Records in the Health Card Registry are linked by a shared family address (civic address where you live) when you apply for a Health Card as a family with your spouse or common-law partner and both of you authorize address updates on each other’s behalf.
Address consent authorization is limited to address updates and doesn’t permit changes to your other personal information.
Once the address consent authorization is registered:
- any address change made by your spouse or common-law partner for a dependant also applies to you and your address
- both you and your spouse or common-law partner can enquire and update information related to your dependants
- your address consent authorization is recorded in the Health Card Registry and remains in the registry until you and your spouse or common-law partner no longer share a family address, you revoke (withdraw) the consent or MSI Resident Services is notified that you moved away from Nova Scotia

If you need your address consent authorization to take effect immediately, contact MSI Resident Services to register your decision.
Changing your consent decision
It’s your choice. You can change your consent decision at any time.
Who can use this form
You can use this form to provide address consent authorization if both you and your spouse or common-law partner:
- are a Canadian citizen or permanent resident
- already have a Health Card
What you need to do
- Complete the form online.
- Submit your completed form.
If you need your consent decision to take effect immediately, contact MSI Resident Services to register your decision.
How long it takes
Your Health Card information is updated right away if you contact MSI Resident Services to register (or change) your address consent authorization. It should take 4 weeks to update your Health Card information if you submit your address consent authorization online or by mail, fax or secure drop box. It can take longer if more information is needed or if your form hasn’t been filled in correctly.
Cost
There is no cost to register address consent authorization.

Make sure you have your spouse or common-law partner available to provide consent or sign the form.

When you submit the form online you need to create an account or sign in.
You can use the Address Consent Form (PDF 145 kB) if you’re unable to complete the form online. Send your completed form by mail or fax. Or drop it off at MSI Resident Services (use the secure drop box; in-person service isn’t available).
You can contact MSI Resident Services if you’re unable to mail, fax, drop off or upload the form.
