Food Safety Complaint Form

If you see or experience inappropriate food handling practices by food workers and/or unhealthy conditions at food establishments in Nova Scotia, please report them here as soon as possible.

The Food Safety Section responds to all complaints in a timely manner. This usually involves a visit to the site in question, assessment of the problem, working with the site management to correct the problem, and education to other staff to avoid the problem in the future.

Did you or others in your party become ill? Time is very important in investigating food-borne illnesses. Please call one of our main offices, Monday to Friday 8:30 a.m. to 4:30 p.m.

You will be briefly interviewed over the phone about what you ate or drank at the food establishment and what symptoms you experienced. All information is kept confidential.

Sometimes there are chains of food establishments with the same name or similar names to other restaurants so to ensure our inspectors are sent to the one you are writing about, we need to know its location.

State the full name of the food establishment. Please do not use abbreviations or nicknames.
