Choose billing option: Seniors’ Pharmacare Program
You need to choose how you want to make payments to the Seniors’ Pharmacare Program.
Use the Billing Choices Form to select which method you want to use to make payments to the Seniors’ Pharmacare Program. You need to complete and submit the form as part of the Seniors’ Pharmacare Program application process if you’re unable to complete the application process online. If you complete the application process online you don’t need to complete and submit the form (the online process includes your billing options).
You need to choose 1 of 3 methods to make billing payments to the Seniors’ Pharmacare Program. Options include:
- getting an invoice and paying each month
- getting an invoice and paying for the whole year
- having payments withdrawn directly from your bank account

You need to use this form to select which payment method you want to use to make payments to the Seniors’ Pharmacare Program if you’re not completing the Seniors’ Pharmacare Program application process online.

- Submit your completed form and supporting documents.

Online
When you submit the form online you need to create an account or sign in. You also need to upload the Billing Choices Form - Seniors’ Pharmacare Program PDF and any supporting documents.
