Canada Revenue Agency income check: Seniors’ Pharmacare Program
You may be eligible for a reduced or waived premium. To see if you’re eligible, you need to agree to have your income verified by Canada Revenue Agency (CRA).
Use the CRA Consent Form to agree to have your income verified by Canada Revenue Agency. You need to complete and submit the form as part of the Seniors’ Pharmacare Program application process if you’re unable to complete the application process online. If you complete the application process online you don’t need to complete and submit the form (the online process includes your CRA consent).
Once you complete the form, the Seniors’ Pharmacare Program gets your income information from the Canada Revenue Agency each year.

- Social Insurance Number for you and your spouse (if applicable)
- your spouse (if applicable) available to sign the form
