If you’re applying for reimbursement through the Pharmacare programs, you need to submit the Prescription Reimbursement Claim Form.

Who can use this form

You need to use this form to submit a claim to the Pharmacare programs.

You need to submit the form within 6 months from the date your prescription was dispensed.

What you need to do

  1. Complete the Prescription Reimbursement Claim Form (Manual Claim) (PDF).
  2. Check the form for details on all required supporting documents.
  3. Submit your completed form and supporting documents.

How long it takes

It should take 4 to 6 weeks to get a reimbursement cheque for the approved amount of your claim. It can take longer if more information is needed or if your application hasn’t been filled in correctly.

Cost

There is no cost to submit the form.

  • Health Card number
  • medical expense report from your pharmacy or official prescription receipts from your pharmacy
