Death Certificates

Vital Statistics issues both short and long form death certificates. To be sure you are requesting the correct one, contact the organization that requires the certificate to confirm what they require.

Short Form Death Certificate

The short form death certificate includes:

  • full name
  • sex
  • date of death
  • age
  • place of death
  • date of registration
  • registration number
  • date of issue

Long Form Death Certificate

The long form death certificate contains all of the information found on the short form certificate, as well as a number of other details. Photographic print of original death registration containing medical cause of death are released only to next of kin, an executor of an estate in selected circumstances, with authorization of the Minister or court order.

The long form death certificate includes:           

  • all the information found on the short form death certificate and the following, if recorded on the death record.
  • residence
  • occupation
  • marital status
  • name of spouse
  • name of father
  • name of mother
  • name of attending physician
  • name of funeral director
  • disposition
  • place of disposition
  • name of informant
  • address of informant
  • relationship to deceased
  • date of registration
  • registration number
  • date of issue

Indicate accents on your certificate

If your name or address contains any French characters or symbols they can be included. If your last name (surname), first name or middle name(s) contain any French characters or symbols as shown on your original birth registration, please indicate this by checking the applicable box on the online application form.

You may also indicate this by answering yes to the question within the online application form.
