You can choose to donate organs and tissues after death. Whether the donation takes place depends on many things, including if there’s a match with someone waiting for a transplant.

If you don’t have a Health Card
If you don’t have a Health Card, you need to contact MSI Resident Services to register your decision.
Who can use this form
You can register your decision to donate your organs and tissues after death if you have a Health Card and are 16 or older. If you’re 15 or younger, your parent or legal guardian needs to complete the registration for you.
What you need to do
- Complete the form online.
- Submit your completed form.
How long it takes
It should take 2 weeks to record your donation decision in the Health Card Registry and 4 to 6 weeks to send you a new Health Card if needed (your decision is displayed on the front of your card). Timing may vary depending on how you register your decision and the donation decision. It can take longer if more information is needed or if your form hasn’t been filled in correctly.
If you change your donation decision and choose not to be a donor (opt out) and register your decision to opt out through the Request to opt out of organ and tissue donation service, it should take 3 to 5 days to record your donation decision in the Health Card Registry and 6 weeks to send you a new Health Card if needed.
Cost
There is no cost to register your decision.

When you submit the form online you need to create an account or sign in.
You can use the Organ and Tissue Donation Form (PDF 152 kB) if you’re unable to complete the form online. Send your completed form by mail or fax. Or drop it off at MSI Resident Services (use the secure drop box; in-person service isn’t available).
You can contact MSI Resident Services if you’re unable to mail, fax or drop off the form.
If you use the PDF to register your decision, make sure you have your Health Card number.
