If you become ill
If you or others in your party become ill after visiting a food establishment, it’s very important to make food safety complaint right away. Time is very important when investigating foodborne illnesses.
You’ll be interviewed over the phone about what you ate or drank at the food establishment and what symptoms you experienced. All information is kept confidential.
Make sure to seek medical attention if you think you may have a serious illness.
Before you start
Make sure you have:
- food establishment name and contact information (sometimes food establishments have similar names, so make sure you have the right name and location)
