Hazard Summary

There were two separate wheel loss incidents in January and February of 2002, where the rear, passenger side, dual wheels separated from the truck. In these cases the vehicle was a truck with a single axle and dual rear wheel.

Cause

In examining the cause of the wheel separation it was determined that four variables could have been responsible:

  • incorrect wheel torque
  • corrosion
  • incorrect tire inflation
  • driving conditions

Corrective Actions

  • Verify the vehicle manufacturer's recommendations/specifications for torqueing procedure.
  • Verify all wheel position tire pressures.
  • Implement a regular retorqueing program.
  • Increase operator awareness of the importance of tire pressure and overall wheel and tire security and condition.
