Vital Statistics Forms Regulations

made under clause 51(1)(a) of the
Vital Statistics Act
R.S.N.S. 1989, c. 494
O.I.C. 73-1340 (December 11, 1973), N.S. Reg. 114/73

1 The form attached hereto bearing the heading "Physician's Notice of a Live Birth or Stillbirth" is prescribed as the form to be used under Section 3 of the Vital Statistics Act.

2 The form attached hereto bearing the heading "Registration of Live Birth" is prescribed as the form to be used under subsection 2 of Section 4 of the Vital Statistics Act.

3 The form attached hereto bearing the heading "Delayed Registration of Birth" is prescribed as the form to be used under Section 5 of the Vital Statistics Act.

4 The form attached hereto bearing the heading "Registration of Stillbirth" is prescribed as the form to be used under subsection 2 of Section 12 of the Vital Statistics Act, and the portion of the said form following the sub-heading "Medical Certificate" is prescribed as the form to be used under subsection 3 of Section 12 of the said Act.

5 The form attached hereto bearing the heading "Registration of Marriage" is prescribed as the form to be used under subsection 2 of Section 15 of the Vital Statistics Act.

6 The form attached hereto bearing the heading "Registration of Death" is prescribed as the form to be used under subsection 2 of Section 17 of the Vital Statistics Act, and the portion of the said form following the sub-heading "Medical Certificate of Death" is prescribed as the form to be used under subsection 3 of Section 17 of the Vital Statistics Act.

7 The form attached hereto bearing the heading "Burial Permit" is prescribed as the form to be used as a burial permit under Sections 12 and 17 of the Vital Statistics Act.

8 No form that has not been provided by the Registrar General shall be used for a purpose for which a form is prescribed by these regulations, provided however that the previous forms prescribed by regulations made by the Governor in Council may continue to be used for the purposes for which the forms had once been prescribed.

9 All regulations heretofore made prescribing forms for the matters enumerated in Sections 1 to 7 of these regulations are hereby rescinded provided however that it is not the intention of the Governor in Council to rescind the regulations made respecting returns of births, stillbirths and deaths occurring in hospitals by the Governor in Council pursuant to the Vital Statistics Act and it is hereby declared that those said regulations do remain in full force and effect.

10 These regulations come into force on the first day of January, 1974.
